For offline, you have to go to the bank and take form. Otherwise, you can fill the form by visiting the Government site (http://pmfby.gov.in).

Required documents for Pradhan Mantri Fasal Bima Yojana

In order to avail of this scheme, the farmer has to give his address, an ID card, whether it is a PAN card, a driving license, a voter ID card or an Aadhar card and an address proof along with it.
If the farm is in the name of the farmer and belongs to him, then add his measles number/account number with the form.
In addition, if the farmer has sown the crop in the field, then you have to present proof of this along with the form. For this, you can write a letter from the village patwari, sarpanch or principals and sign them. This plan will only be compensated by losses due to natural calamities.
